Give to the surgery you use

There is no office to pay for and nobody here draws a wage. What you give buys equipment for Constantine Surgery, and it stays in the village.

We do not take card payments on this website, so no platform takes a cut on the way through. Email us and we will send you everything you need, by return.

Three ways to give

  • Once

    A single gift

Any amount, whenever suits you. Most of what we buy is paid for out of many small gifts rather than one large one.

    Email us to give once

We will reply with our bank details, or arrange a cheque

  • Monthly

    A standing gift

A few pounds a month lets us commit to a piece of equipment before we have the whole sum in the bank.

    Email us to give monthly

We will send you a standing order form to sign

  • In your will

    A gift in your will

A legacy to the Friends stays in the practice that looked after you. Your solicitor will need our registered name and charity number, both at the foot of this page.

Gift Aid

If you pay UK tax, Gift Aid adds 25p to every £1 you give — at no cost to you

We claim it back from HMRC, not from you. All we need is your name, your home address and a tick to confirm you are a UK taxpayer. Because your gift comes straight to us, we claim the Gift Aid ourselves and every penny of it stays with the practice. Tell us if you stop paying UK tax and we will stop claiming.

Gift Aid declaration — what the paper looks like

Nothing to fill in here. This is a picture of the form we will send you.

I want to Gift Aid this donation and any donations I make in the future or have made in the past 4 years to The Friends of Constantine Surgery.

I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year, it is my responsibility to pay any difference.

Full name
Home address
Postcode

Ask us for a declaration and we will email you one to sign and return, or hand you one at the surgery. If you pay UK tax it is worth doing: it adds a quarter to everything you have given us in the last four years.

Other ways to help

Money is not the only thing we need

Help at a village event

Fetes, coffee mornings and stalls only happen because people turn up early and stay late. Tell us you are willing and we will put you on the list.

Use the tin at the surgery

There is a collection box in the surgery. Change adds up faster than people expect.

Remember us in your will

Ask your solicitor to name The Friends of Constantine Surgery, registered charity 293378.
